The Geriatric Care Nurse Practitioner (NP) plays a pivotal role in delivering primary health care and supportive services to elderly clients and their families within the community setting. This position reports directly to the Director of Long Term Care & Community Support Services.

The NP is an integral member of a multidisciplinary team dedicated to the Home Dementia Care Initiative, a project aimed at enhancing support for individuals experiencing moderate to advanced stages of dementia.

Key responsibilities include conducting comprehensive geriatric evaluations in the community and implementing strategies to mitigate falls, prevent delirium, and ensure effective management of chronic health conditions in a home environment.

The NP will tailor care approaches and utilize technology based on the unique needs of clients, considering their social, economic, and cultural contexts.

In this role, the NP will serve as a practitioner, consultant, educator, leader, and researcher, collaborating closely with the local geriatric care team.

Core Responsibilities:

  • Deliver safe, effective, and high-quality care to geriatric clients with dementia in a home-based setting.
  • Advocate for clients within the healthcare system, ensuring their needs are met in primary care.
  • Collaborate with healthcare providers and community partners to enhance geriatric dementia care.
  • Assist in the development and implementation of best practices in geriatric primary care for dementia.
  • Maintain regular communication with the lead physician and other professionals involved in the initiative.

The ideal candidate will demonstrate a commitment to person- and family-centered care, prioritize safety, and work collaboratively with all team members to enhance patient and worker safety by identifying risks and implementing improvements.

Qualifications:

  • Graduation from an accredited nurse practitioner program.
  • Current Basic Life Support (BLS) certification for healthcare providers.
  • Strong collaboration, clinical reasoning, and interpersonal skills.
  • Proven organizational and time management abilities.
  • Proficiency in electronic health records and database management.
  • Consistent demonstration of the core values of Central Health.

Additional Requirements:

  • Possession of a valid driver's license and access to a private vehicle for client visits.
  • A satisfactory record of work performance and attendance.
  • A clear Certificate of Conduct from the appropriate authorities.

Hours of work: 75 hours bi-weekly (Shift work may be required). Central Health is committed to being an equal opportunity employer.
