Job Overview:
The Licensed Practical Nurse (LPN) at Phoenix Society collaborates with an interprofessional team to deliver healthcare services to clients facing physical and/or mental health challenges, substance dependence, concurrent disorders, and chronic illnesses. The LPN is tasked with the planning, execution, and assessment of nursing care within a collaborative clinical framework, adhering to the Standards of Practice set forth by the British Columbia College of Nurses and Midwives (BCCNM), while aligning with the mission, vision, and values of Phoenix Society. This role involves a full-time rotating schedule (06:00-18:00, 18:00-06:00).
Key Responsibilities:
Client Care:
Support physicians and team members during various examinations, treatments, and diagnostic procedures. Execute a range of nursing tasks including monitoring vital signs and conducting urine tests as instructed. Relay patient information to the appropriate supervisor, physicians, and team members. Assist in the care of clients with unpredictable outcomes or complex needs requiring advanced knowledge and skills. Contribute to the formulation and adjustment of personalized care plans by identifying client priorities and suggesting necessary modifications. Monitor client status against expected outcomes and evaluate the effectiveness of nursing interventions, reporting any unexpected changes to the supervisor. Assess clients' strengths and limitations, addressing their physiological, psychological, social, and spiritual needs. Collaborate with the treatment team to support clients in meeting their program requirements through monitoring and motivational interviewing. Review medication administration records and consult with supervisors or physicians regarding any concerns. Administer medications in accordance with care plans and physician directives. Ensure proper storage of client medications as per policies and procedures. Establish and maintain therapeutic relationships with clients.Administrative Duties:
Maintain accurate computerized and paper records, tracking results and documenting consultations appropriately.Health & Safety:
Stay informed about current Occupational Health & Safety guidelines and standards. Utilize supplies and equipment in line with organizational standards and safety protocols. Manage hazardous materials and sharps according to established procedures. Identify and report unsafe practices or professional conduct in line with organizational policies. Participate in incident investigations as necessary. Respond to incidents, providing nursing care during emergencies and managing injuries or aggressive behaviors. Provide first aid in accordance with Occupational First Aid Level 1 standards. Complete necessary reports for first aid and incidents. Assess individual client risk factors and integrate risk mitigation strategies into care plans to enhance safety for staff and clients.Qualifications:
Post-secondary diploma in nursing or a related field (2 to 3 years). Current registration as a Registered Nurse or Registered Psychiatric Nurse with the BCCNM. 1 to 3 years of experience in trauma-informed acute or community health nursing care for clients with physical and psychiatric conditions. Successful completion of a criminal records check with vulnerable sector screening. Completion of Tuberculosis Screening and Employee Immunization Record forms. First Aid Level 1 and current CPR certification. Valid driver's license.Skills & Knowledge:
Strong understanding of community health nursing theory, including mental health and addictions. Familiarity with BCCNM's Standards for Nursing Practice. Knowledge of the BC Mental Health Act and available community resources. Proficiency in conflict resolution and mediation techniques. Competence in using computerized patient care information systems and office software. Strong case management and clinical support skills. Excellent communication skills, both written and verbal.
