Community Integration Specialist

The Community Integration Specialist plays a crucial role in assessing, supporting, and guiding clients at the Restigouche Hospital Center. This position focuses on fostering client autonomy in accordance with the interdisciplinary intervention strategy, aiming to facilitate their reintegration into society through the execution of treatment and rehabilitation initiatives. The responsibilities encompass a variety of intervention techniques, including individual client consultations, support for therapeutic groups led by nursing professionals, and the organization of educational sessions.
KEY RESPONSIBILITIES:
- Conduct evaluations and provide assistance to clients in their journey towards autonomy;
- Implement treatment and rehabilitation programs tailored to individual needs;
- Facilitate daily individual meetings with clients to monitor progress;
- Support therapeutic groups and educational sessions to enhance client engagement;
- Ensure continuity of care through organized scheduling and service delivery;
- Adhere to clinical principles of recovery, risk assessment, and responsiveness;
- Collaborate within an interdisciplinary clinical framework to achieve optimal outcomes;
QUALIFICATIONS:
- High school diploma or equivalent, supplemented by a community integration program or course of six months to one year;
- Current certification in Basic Life Support (BLS);
- Training in Non-Violent Crisis Intervention (CPI) - Advanced techniques or willingness to complete training;
- Training in Suicide Prevention and Management or commitment to undertake training;
- Valid driver's license in accordance with New Brunswick Motor Vehicle Act;
- Strong judgment and organizational capabilities;
- Ability to employ diverse intervention strategies creatively;
- Competence to work autonomously as well as collaboratively;
- Physical capability to fulfill assigned duties;
- Positive prior work history regarding performance and attendance;
- Commitment to professional ethics and organizational values;
- Respect for confidentiality protocols of the Vitalité Health Network;
- The selection process may include assessments through oral, written, or practical evaluations;
- The employer retains the discretion to modify the duration of temporary roles for operational needs;
