About the Role

We are seeking a highly organized and detail-oriented individual to join our team as a Care Coordinator at Hawthorne Place Care Centre Facility Inc.

Key Responsibilities
  • Coordinate the flow of information and maintain accurate records
  • Manage incoming mail and other materials, and maintain a filing system
  • Order supplies and maintain inventory
  • Enter and format medical reports and correspondence, and prepare spreadsheets and documents for review
  • Initiate and maintain confidential medical files and records
  • Perform data entry and provide customer service
  • Schedule staff and resolve conflict situations
  • Perform reception and clerical duties
  • Employ creative problem solving to identify opportunities for process improvement
Requirements
  • Secondary (high) school graduation certificate
  • 3 years to less than 5 years of experience
  • Ability to work in a nursing home/home for the aged environment
  • Excellent communication and organizational skills
