Job Title: Care Coordinator

Job Summary:

The Care Coordinator will play a vital role in assessing patient care needs, determining eligibility for services, and developing individual care and service plans. This is an exciting opportunity to work in a dynamic and collaborative environment, partnering with patients, caregivers, and healthcare practitioners to deliver responsive, accessible, and integrated care.

Key Responsibilities:
  • Assess patient care needs and determine eligibility for services
  • Develop individual care and service plans in collaboration with patients and their families
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families, promoting patient-centered care and empowerment
  • Balance patient needs and choices with available resources, ensuring patients' values and preferences are respected
Requirements:
  • Membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.)
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports, and forms
  • A valid driver's license and access to a reliable vehicle
  • Proficient in a Windows environment
Preferred Qualifications:
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
