Job Title: Care Coordinator

Job Summary:

The Care Coordinator will play a vital role in assessing patient care needs, determining eligibility for services, and developing individual care and service plans. This is an excellent opportunity for a motivated and compassionate individual to make a positive impact in the lives of our patients.

Key Responsibilities:
  • Collaborate with patients and their families to assess care needs and develop individual care plans
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources
Requirements:
  • Fluency in both French and English
  • Membership in good standing with the applicable regulatory body
  • 1+ years of recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
