Job Summary

The Care Manager is a key member of the interdisciplinary care team, responsible for leading the team in providing high-quality, resident-centered care. This role requires strong leadership and communication skills, as well as the ability to work collaboratively with other healthcare professionals to ensure the best possible outcomes for residents.

Key Responsibilities
  • Recruit, lead, and supervise the interdisciplinary team to ensure effective care planning and delivery.
  • Conduct holistic assessments of residents and families to inform individualized care plans.
  • Provide leadership and guidance to the care team, promoting a culture of safety and resident-centered care.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
  • Develop and implement quality improvement initiatives to enhance resident care and services.
  • Ensure compliance with regulatory requirements and PPSL policies and procedures.
Requirements
  • Completion of a Bachelor's degree in Nursing and/or Psychiatric Nursing.
  • C.R.N.A registration in good standing.
  • 3 years of recent related experience or an equivalent combination of education, training, and experience.
  • Current CPR and First Aid certification.
  • Mental Health experience is an asset.
  • Clear criminal record security clearance with Vulnerable Sector included.
  • Current practicing registration with the applicable provincial registering body.

  • Problem-solving and critical thinking skills.
  • Ability to prioritize tasks and manage multiple responsibilities.
  • Knowledge of mental health and gerontology principles and best practices.
