McClendon Center is a well-regarded and highly rated agency within the District of Columbia. It is also the sole independent mental health organization in the District accredited by the Joint Commission on Accreditation of Healthcare Organizations.

Position Overview

KEY RESPONSIBILITIES:

  1. Conducts continuous evaluations of clients' preferences and clearly defined needs, facilitating the creation and revision of service plans and assessments every 180 days without interruptions.
  2. Provides assistance and support to clients during crisis situations.
  3. Guides clients in applying for various benefits and entitlements, including SSI, Medicaid, Section 8, energy assistance, and other eligible services.
  4. Helps clients identify and utilize community resources, such as self-help groups and recreational facilities.
  5. Assists in finding safe, sanitary, and affordable housing suitable for the required level of care and offers support during transitions between housing types.
  6. Accompanies clients to essential services when necessary, particularly for medication and psychiatric appointments.
  7. Educates clients on using public transportation and applying for reduced fare options.
  8. Supports clients in managing their symptoms, monitors adherence to medication, and addresses any side effects or medication-related issues.
  9. Facilitates access to health-related services as required.
  10. Provides education, support, and consultation to clients' families and support networks as appropriate.
  11. Documents all services rendered through billing and progress notes by the end of the next business day following the service date.
  12. Reviews and updates assessments and service plans every 180 days without gaps.
  13. Maintains records of all client-related correspondence and activities as they occur.
  14. Completes crisis plans, Level of Care Utilization Score (LOCUS), Global Assessment of Individualized Needs (GAIN) assessment, and Reaching Recovery assessment with clients every 180 days.
  15. Offers external monitoring and support when clients are engaged with other service providers.
  16. Participates in an on-call system to respond to clients after hours, during holidays, and on weekends.
  17. Advocates for clients as they navigate social, governmental, and community systems.
  18. Engages in staff meetings, case conferences, and training sessions as necessary.
  19. Collaborates effectively with all associated agencies, demonstrating positive interactions.
  20. From the sixth week of employment, bills an average of 5.00 hours per day for each day worked; productivity is calculated and reported weekly. Productivity will be adjusted for any approved absences. (Failure to meet the weekly productivity standard for four consecutive weeks in a calendar year or for more than seven weeks in a calendar year may lead to disciplinary action.)
  21. Maintains confidentiality of clients at all times and adheres to the Code of Ethics of the United States Psychiatric Rehabilitation Association.
  22. Gains familiarity with Joint Commission, Medicaid, and Department of Behavioral Health (DBH) standards and compliance requirements, assisting as needed in maintaining Joint Commission accreditation, Medicaid compliance, and DBH.
  23. May perform additional duties as assigned.
Qualifications
  • High school diploma is required.
  • One (1) to four (4) years of experience in a social services or nonprofit setting. Experience in a mental health or similar organization is highly preferred.
  • Ability to work flexible hours as needed, including evenings and weekends.
SKILLS AND KNOWLEDGE REQUIREMENTS:
  • Ability to collaborate with a multi-disciplinary team within an outpatient mental health organization.
  • Exceptional communication skills, capable of sharing information clearly and concisely, listening actively, and respecting the abilities of others.
  • Understanding of ethical practices and principles related to the care of mental health clients.
  • Strong written and verbal communication abilities.
  • Proficient in computer skills, including the use of Microsoft Office Suite.
  • Knowledge and experience navigating an Electronic Medical Record (EMR), including data entry.
  • Ability to relate to adults diagnosed with serious and persistent mental illness, applying knowledge of mental health concepts and theories.
  • Ability to function effectively in an environment serving adults with serious and persistent mental health issues.
  • Commitment to maintaining client confidentiality.
  • Exhibits strong organizational skills and the ability to prioritize and manage multiple tasks independently.
