We are seeking a highly skilled and experienced Patient Care Coordinator to join our team at KMH Cardiology Centres Inc. in Mississauga. As a Patient Care Coordinator, you will play a vital role in facilitating timely and high-quality care for patients attending our clinics.
Key Responsibilities- Coordination of In-Clinic Operations: Ensure seamless coordination of in-clinic operations for physicians, including management of physician orders and tasks in a timely manner.
- Patient Enquiries: Respond to patient enquiries regarding clinical care within 24 business hours.
- Test and Procedure Information: Inform and educate patients about tests and procedures as requested by physicians.
- Prescription Renewal Requests: Process prescription renewal requests as directed by physicians in collaboration with administrative staff within 24 business hours.
- External Facility Liaison: Liaise with external facilities on patient referrals on a timely basis.
- Utilization of Tableau Reports: Utilize Tableau reports for daily workflow.
- Cross-Functional Team Collaboration: Collaborate with cross-functional teams and other KMH staff to assist with patient care.
- Collegial Clinic Environment: Foster a collegial, respectful clinic environment.
- Proven Experience: Proven ability to work proficiently in a busy, fast-paced specialist clinic environment.
- Similar Experience: Similar experience in administration and a clinic setting is preferred.
- Medical Terminology: Strong knowledge of medical terminology.
- Technology Proficiency: Proficiency in technology, including MS Office and EMR systems experience.
- Time Management and Organization: Excellent time management and organizational skills.
- Availability: The ability to provide coverage at KMH Hamilton is an asset.

What We Offer- Contract Length: 12-month contract with full-time hours.
- Work Schedule: 5 hours per week on-site during the hours of 8:00 AM - 4:00 PM, Monday to Friday, with Saturday's required on rotation with the team for 1-2 times per month.
