Job Summary

The Advanced Practice Nurse will utilize advanced clinical practice skills to provide direct clinical care, education, research, consultation, and treatment to support the care needs of designated patients. The successful candidate will function as an independent practitioner and as a collaborative partner within the inter-professional team.

Key Responsibilities
  • Conducting and documenting medical history and physical exam findings in the Electronic Medical Record (EMR)
  • Diagnosing medical conditions and developing treatment plans
  • Ordering and interpreting laboratory and diagnostic imaging results
  • Prescribing pharmacologic and non-pharmacologic therapies independently or in consultation with physician partners
  • Collaborating with the Hospitalists, Specialists, Community Primary Care Practitioners, and Allied Health team members
  • Utilizing diagnostic reasoning and decision-making processes to formulate medical diagnoses and treatment plans
  • Initiating, tailoring, and evaluating medical and nursing therapy
  • Performing specialty-specific diagnostic and therapeutic interventions in accordance with scope of practice and established practice guidelines
  • Supervising and precepting nursing students and other interdisciplinary students
Requirements

**Education and Certification**

  • Current registration with the College of Nurses of Ontario is required
  • BScN required
  • Nurse Practitioner Certification required
  • Masters degree in Nursing/Nurse Practitioner Stream is preferred
  • Acute Care Nurse Practitioner Certification preferred
  • Gerontology Nursing Certification is preferred
  • BCLS is required

**Experience**

  • Minimum three (3) years experience in advanced practice nursing in adult medicine/geriatrics/eldercare preferred
  • Minimum three (3) years relevant clinical specialty-related experience as a Registered Nurse required, experience in Acute Care preferred
  • Recent experience in a leadership role within an inter-professional team preferred
  • Relevant knowledge of inpatient care environment preferred
