We are seeking a highly skilled and compassionate Nurse Case Manager to join our team at CB Canada. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support to individuals with complex medical needs.
Key Responsibilities:
- Provide Medical Support: Offer medical guidance and support to patients and healthcare providers, including education on specific diseases and therapies, and disease management.
- Telephone Support: Provide telephone support, including maintenance calls and adherence support, as needed.
- Database Management: Manage and maintain databases, ensuring accurate and up-to-date information is captured and recorded.
- Case Management Protocols: Administer and adhere to case-management protocols, including clinical monitoring, data collection, patient follow-up, compliance, and adverse event reporting.
- Collaboration and Communication: Work closely with healthcare providers, pharmacies, and other agencies to coordinate patient care and resolve issues as they arise.
- Training and Development: Participate in ongoing training and development opportunities to enhance skills and knowledge in case management and patient support.
- Leadership and Teamwork: Demonstrate strong leadership and teamwork skills, working collaboratively with colleagues to achieve shared goals and objectives.
Requirements:
- Education: Bachelor's degree in Nursing or related field, with a minimum of five years of nursing experience.
- Experience: A minimum of two years of experience in public or private third-party reimbursement, pharmaceutical industry sales, managed care, or clinical support.
- Skills: Strong patient focus, ability to multi-task, work under pressure, and communicate effectively with patients, healthcare providers, and colleagues.
- Language: Bilingualism (French and English) is required.
