Job Summary

We are seeking a highly skilled and compassionate Nurse Case Manager to join our team at CB Canada. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support to individuals with complex medical needs.

Key Responsibilities
  • Provide Medical Support: Offer medical guidance and support to patients and healthcare providers, including education on specific diseases and therapies, and disease management.
  • Telephone Support: Provide telephone support to patients and healthcare providers, including maintenance calls and adherence support as needed.
  • Database Management: Manage and maintain accurate and up-to-date patient information and data in our database.
  • Case Management: Administer and adhere to case-management protocols, including clinical monitoring, data collection, on-going patient follow-up, compliance, therapy disruption/discontinuation monitoring, adverse event reporting, and liaison with primary healthcare providers.
  • Training and Development: Participate in extensive, specialized training sessions for specific therapies provided by our pharmaceutical team.
  • Reimbursement and Funding: Work directly with our reimbursement specialist and physician to secure funding for patient therapies and assist with alternative funding searches when needed.
  • Communication and Collaboration: Establish and maintain effective communication with our team, clients, and healthcare providers to ensure seamless patient care and support.
  • Problem-Solving and Critical Thinking: Utilize strong problem-solving and critical thinking skills to resolve complex patient care issues and make informed decisions.
Requirements
  • Education and Experience: Bachelor's degree or registered nurse (RN) diploma, advanced degree, or specific practice management experience preferred, with five (5) years of nursing experience.
  • Relevant Experience: Minimum of two (2) years in public or private third-party reimbursement arena or pharmaceutical industry in sales (not required, but an asset), managed care, or clinical support, or related experience.
  • Language Skills: Bilingualism (French and English).
  • Skills and Abilities: Strong patient focus, ability to multi-task, work under pressure, provide superior customer service, and possess strong verbal and written communication skills, problem-solving skills, and teamwork abilities.
