Job Summary

The Licensed Practical/Registered Practical Nurse is a key member of our home care team, responsible for providing direct skilled client care under the supervision of a registered nurse. This role involves following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes.

Responsibilities
  • Direct Patient Care: Provide high-quality, patient-centered care to clients in their homes, focusing on their physical, emotional, and social needs.
  • Documentation: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
  • Ongoing Assessments: Conduct regular assessments to identify changes in patient condition and adjust care plans accordingly.
  • Medication Administration: Prepare, administer, and document medications as prescribed by healthcare professionals.
  • Skills and Treatments: Perform all skills, treatments, and procedures competently in accordance with the provincial scope of practice.
  • Infection Control: Follow principles of infection control and Universal Precautions to ensure a safe environment for patients and staff.
Qualifications
  • Certification and Licensure: Current certificate of competence and good standing as a Licensed Practical/Registered Practical Nurse.
  • CPR Certification: Certified in CPR and able to respond to emergencies.
  • Clinical Skills: Strong clinical skills and ability to work independently with minimal supervision.
  • Customer Service: Excellent customer service skills and ability to communicate effectively with patients, families, and healthcare professionals.
