Job Summary:As a Geriatric Social Worker, you will work closely with older adults and their families to provide social work services, promote health, and support independence.
Key Responsibilities:- Conduct comprehensive assessments for older adults and their families to identify social, emotional, and psychological needs.
- Develop and implement client care plans in collaboration with the interdisciplinary team to promote optimal health and well-being.
- Provide education and information sessions to clients, family members, and healthcare staff to promote self-management and advocacy.
- Participate in quality improvement and risk management activities to ensure high-quality care.
- Assist in developing and monitoring policies and procedures for the Senior's Clinic to ensure compliance with regulatory requirements.
- Collaborate with interdisciplinary team members on research and special projects to improve care outcomes.
- Master's degree in Social Work from an accredited institution.
- Two years' recent related clinical experience working with older adults and their families.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
- Demonstrated knowledge and understanding of counseling and therapy skills and clinical practice models relevant to working with older adults.
- Ability to communicate effectively, both verbally and in writing.
- Ability to work independently and as a member of an interdisciplinary team.
- Ability to develop and maintain rapport with clients, interdisciplinary team members, and other stakeholders.
- Ability to plan, problem solve, organize, and prioritize work in a continually changing environment.
