About This Opportunity

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Community Care Specialist. In this role, you will have the opportunity to provide high-quality nursing care to patients in their homes, working with patients of all ages and complexity of care needs.

Key Responsibilities

  • Participate in scheduled visits in the community to assess and address patient needs within your scope of practice.
  • Provide safe and effective nursing care, using appropriate assessment tools and techniques to identify patient needs.
  • Develop and implement individualized care plans, monitoring patient progress and making adjustments as needed.
  • Build strong relationships with patients and their families, using a collaborative approach to care.
  • Maintain accurate and up-to-date patient records, documenting all patient encounters and care plans.

Requirements

  • Registration with the NSCN as a Licensed Practical Nurse in good standing.
  • Valid driver's license and access to a reliable vehicle.
  • Experience in delivering community care is an asset.
  • Reliability, flexibility, and a strong commitment to patient care.
  • Ability to travel to and from patient residences within a designated area.
  • Experience working with vulnerable populations is an asset.
