Vancouver Coastal Health is seeking a skilled Social Worker to join our Overdose Outreach team in Vancouver, BC. As a Social Worker with our organization, you will play a vital role in providing clinical and consultative social work services to clients and families.
Key Responsibilities- Conduct social assessments and provide counseling services to clients and families.
- Develop and implement individualized care plans to address the unique needs of clients.
- Collaborate with interdisciplinary teams to provide comprehensive care and support to clients.
- Maintain accurate and up-to-date client records.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
- Current full registration with the British Columbia College of Social Workers.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two (2) years' recent related experience working with marginalized clients with multiple health issues (HIV and mental health and substance use issues) in community-based or Indigenous-led or First Nations organizations.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
- Basic knowledge of systems thinking as it relates to social work process.
- Working knowledge of counseling skills.
- Understanding of local (Downtown Eastside) and other regional initiatives that may affect patient care, resource access, or practice guidelines in this local clinic setting.
- General knowledge of Trauma-Informed Practice and Harm Reduction approaches and Strengths-based care.
- Demonstrated ability to promote Indigenous Cultural Safety.
- Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations.
