Position Overview

As a valued member of VON Canada, you will play a crucial role in delivering essential support services to clients in their residences and within the community.

Employment Details:

Status: Regular, Full Time (1.0 FTE)
Program: Home Support
Hours: 75 hours bi-weekly
Schedule: Days, Evenings, Weekends
Union: SEIU
On Call: No

Key Responsibilities:

  • Assess and monitor the client's living environment and personal circumstances to identify necessary adjustments in the support plan.
  • Collaborate with the Personal Support Program Manager to develop and update personalized care plans based on comprehensive assessments.
  • Implement modifications to care plans as directed, ensuring safety and compliance with relevant regulations.
  • Communicate any changes in client needs or home conditions to the appropriate supervisory personnel.
  • Perform various personal support tasks tailored to the client's requirements, including light housekeeping, meal preparation, and shopping assistance.
  • Assist clients with personal care activities, such as mobility support, hygiene, and dressing, under the guidance of supervisory staff.
  • Provide basic bedside care as directed by nursing professionals.
  • Offer respite care services to clients and their families as needed.
  • Support families by providing childcare services to facilitate relief for caregivers.
  • Organize daily activities and prioritize tasks in alignment with the care plan.
  • Complete necessary documentation and reports to ensure effective service delivery.
  • Evaluate and adapt communication strategies to meet the client's needs.
  • Engage with healthcare team members to address the holistic needs of clients and report any concerns to supervisors.
  • Adhere to occupational health and safety standards, as well as VON policies and procedures.
  • Fulfill additional responsibilities as assigned.

Qualifications:

  • High school diploma; secondary education preferred.
  • Certification in Home Support Worker or equivalent required.
  • Current Standard First Aid or CPR certification, as applicable.
  • Experience in household management and working with vulnerable populations.

Skills Required:

  • Excellent verbal and written communication abilities.
  • Strong interpersonal skills to engage effectively with clients and families.
  • Good observational and assessment capabilities.
  • Knowledge of community resources and support services.
  • Ability to work collaboratively within a multidisciplinary team or independently.
  • Capacity to maintain confidentiality and handle sensitive information.

Additional Requirements:

  • Access to a personal vehicle for work-related travel.
  • Current driver's license and proof of vehicle insurance.
  • Completion of a satisfactory Criminal Records Check.
  • Availability for flexible working hours.
  • Compliance with Personal Protective Equipment (PPE) requirements.
