We are seeking a highly skilled and dedicated Registered Practical Nurse to join our interdisciplinary team at OSMH. As a key member of our healthcare team, you will be responsible for providing direct patient care, developing and implementing care plans, and coordinating patient services in accordance with our hospital's mission, vision, and policies.
Key Responsibilities- Provide high-quality patient care, including assessment, planning, implementation, and evaluation of patient care plans.
- Develop and maintain effective relationships with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in interdisciplinary team meetings to discuss patient care plans and coordinate services.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Contribute to a positive and supportive work environment by promoting a culture of respect, empathy, and open communication.
- Current general class registration with the College of Nurses of Ontario (CNO) is required.
- Minimum two (2) years acute care experience is required.
- Experience in rehabilitation, seniors care, and/or palliative care is preferred.
- Member of the IV Team or willingness to acquire within two (2) weeks of hire is required.
- Current certification in BCLS is required.
- Ability to work all shifts, weekdays, weekends, and holidays is required.
- Excellent organizational skills, professionalism, respect for confidentiality, and flexibility to adapt to crisis situations and change are essential.
- Willingness to complete all education required for employment on the unit within 6 months of hire is required.
- Previous work experience on an Acute Stroke Unit or Integrated Stroke Unit is preferred.
- Certificate in Gerontology and/or Palliative Care from a recognized college is considered an asset.
- Excellent written and verbal communication skills are required.
- Excellent work performance and attendance record is required.
