Home Management Consultant

The Home Management Consultant is a dynamic and innovative role that plays a critical part in promoting a welcoming community for individuals with intellectual disabilities and their families. As a leader in the field, Community Living Toronto offers a wide range of supports to over 6,000 individuals, and this position is key to delivering those services.
Key Responsibilities- Deliver In-Home Consultations: Conduct assessments, provide skill-building, informal counseling, and behavior consultation to enhance a family's ability to meet the needs of a child with developmental disabilities.
- Advocate and Collaborate: Advocate on behalf of families, act as a liaison, and collaborate with internal and external resources and supports to enhance in-home programs and strategies.
- Develop Person-Directed Plans: Handle requests for Person-Directed Plans from Developmental Services Ontario (DSO) and coordinate the development of plans that help individuals with a developmental disability set goals, make choices, and participate in their communities.
- Coordinate External Supports: Coordinate the provision of necessary external supports and services to assist families and facilitate progress toward specific goals.
- Assess and Evaluate: Assess the effectiveness of strategies and determine when to make necessary changes.
- Supervise and Train: Supervise placements for students completing Bachelor of Social Work and Master of Social Work programs and develop and facilitate internal and external training workshops.
- Education: Bachelor's degree in Social Sciences or a related field, with post-degree specialized study in behavioral science.
- Experience: A minimum of 3-5 years of experience conducting assessments, designing intervention strategies, and working with families and children with developmental disabilities.
- Skills: Knowledge of family systems theory, child development, behavioral strategies, and strength-based approaches, as well as proficiency in computer software, including Microsoft Office.
This role requires occasional moderate sensory demands and physical demands, including data entry, Person-Directed Plan development, and maintaining files and case notes.
Key Relationships- Families and Individuals Being Served: Conduct assessments, interventions, and planning, and utilize skills in influencing and persuading to build relationships.
- External Agencies and Community Partners: Gather information, advocate on behalf of individuals and their families, and collaborate with other professionals.
- Developmental Services Office: Provide updates and manage requests for Person-Directed Plans.
- Inter-Agency Staff: Provide and obtain training, program recommendations, and services.
