About the Role
In this role, you will be part of a specialized program that delivers healthcare to individuals in correctional facilities. Your work will help ensure that every individual has access to necessary healthcare services, fostering an environment of healing and support.
Key Responsibilities
- Engage with healthcare professionals within the correctional facility to discuss the anticipated needs of clients nearing release.
- Collaborate with the inter-professional healthcare team and clients to gather and validate health information, assessing their needs for community health services.
- Coordinate with community resources to ensure seamless transitions for clients, providing them with the necessary support services.
- Facilitate connections to various community programs and services, ensuring comprehensive transition plans are in place.
- Participate in ongoing team development and quality improvement initiatives to enhance care delivery.
Qualifications
- Graduation from an accredited School of Nursing with current registration as a Registered Nurse or Registered Psychiatric Nurse.
- A minimum of two years of relevant experience in acute care, primary care, or community health nursing.
Skills and Competencies
- Ability to foster partnerships with external organizations to maintain continuity of care.
- Proficient in applying nursing theories and practices.
- Strong understanding of primary healthcare services and philosophies.
- Effective communication skills, both verbal and written.
- Commitment to understanding and addressing legislative obligations and provincial commitments related to Indigenous health and reconciliation.
