Client Care Navigator

At Vision Loss Rehabilitation Canada, we are seeking a highly skilled and compassionate Client Care Navigator to join our team. As a key member of our healthcare team, you will play a vital role in coordinating and integrating supports and services around individuals living with vision loss.
Key Responsibilities:- Assess client needs and develop personalized care plans to achieve optimal outcomes.
- Coordinate and facilitate linkages between clients, healthcare providers, and community services to ensure seamless care delivery.
- Provide education and support to clients and their families to enhance their understanding of vision loss and available resources.
- Collaborate with multidisciplinary teams to ensure comprehensive care and address complex client needs.
- Monitor and evaluate client progress, making adjustments to care plans as needed.
- 3-5 years of experience in client services, preferably in the healthcare or social services sector.
- Experience working with individuals with vision loss and their families.
- Strong knowledge of community resources, healthcare systems, and clinical practices.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team.
- Bilingualism (French and English) is an asset.
