Job Title: Outpatient Social Work

University Health Network is seeking a highly skilled and compassionate Outpatient Social Work professional to join our team. As a key member of our inter-professional team, you will play a vital role in providing high-quality patient care and support.

Job Summary:

The successful candidate will be responsible for assessing and evaluating patients for readiness for multi-organ transplant from a social support perspective. You will also assess patients to evaluate the need for service, formulate recommendations or treatment plans, and implement intervention plans with patients, family members, and the inter-professional team.

Key Responsibilities:
  • Assess patients for readiness for multi-organ transplant from a social support perspective
  • Evaluate patients to determine the need for service and formulate recommendations or treatment plans
  • Implement intervention plans with patients, family members, and the inter-professional team
  • Reassess and modify intervention plans as required
  • Liaise with community agencies to provide support and services to patients
  • Complete applications for Alternate Level of Care (ALC) facilities and establish contact with members of the inter-professional team and community agencies
  • Render services for the immediate benefit of the Hospital and the community
  • Provide clinical teaching to Social Work Students, Medical Students, and Professional Colleagues
  • Participate in relevant research activities and projects
Qualifications:
  • Master's Degree in Social Work
  • Registration with the Ontario College of Social Workers and Social Service Workers
  • Membership in the Ontario Association of Social Workers (preferred)
  • Three years of experience working with an inter-professional team in a hospital setting (preferred)
  • Experience in acute care (preferred)
  • Excellent verbal and written communication skills
  • Excellent interpersonal, time management, and organizational skills
  • Excellent problem-solving skills
  • Research experience/education (an asset)
  • Student education/supervision (an asset)
  • Knowledge of policies, legislation, programs, and issues related to social welfare institutions and services
  • Knowledge of a second language (an asset)
