We're seeking a dedicated and compassionate Community Nurse to join our team at Saint Elizabeth Health Care. As a Community Nurse, you will play a vital role in providing high-quality patient care and making a meaningful difference in the lives of our clients.
About the RoleThis is a challenging and rewarding opportunity for a skilled and empathetic nurse to work in a community setting. You will be responsible for providing comprehensive client assessments, developing care plans, and implementing and evaluating care based on best evidence and practices.
Key Responsibilities- Complete comprehensive client assessments and develop personalized care plans
- Implement and evaluate care based on best evidence and practices
- Support clients through care transitions by collaborating with other care team members and community support services

Requirements- Current registration with the College of Nurses of Ontario
- Canadian Nurses Association (CNA) specialty certification an asset
- Enthusiasm and love of community care and passion for evidence-informed practice and excellence in care
- A current CPR certificate is required
- A valid driver's license and vehicle (travel required)
We are offering a Return of Service incentive to eligible nurses who commit to 24 months of employment at a full-time work schedule offered by the employer.
