Community Health Nurse

We are seeking a skilled and compassionate Community Nurse to join our team at Saint Elizabeth Health Care. As a Community Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes and communities.
Key Responsibilities- Conduct comprehensive client assessments and develop personalized care plans to meet the unique needs of each individual.
- Provide skilled nursing care and support to clients in their homes and communities, including wound care, medication management, and education.
- Collaborate with other healthcare professionals and community resources to ensure seamless care coordination and support.
- Document client progress and outcomes, and participate in quality improvement initiatives to enhance patient care and services.
- Current registration with the College of Nurses of Ontario.
- Canadian Nurses Association (CNA) specialty certification an asset.
- Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care.
- A current CPR certificate is required.
- A valid driver's license and vehicle (travel required).
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
