About the Role

VON Canada is seeking a skilled Licensed Practical Nurse to join our team in providing high-quality home care services to our clients. As a LPN, you will play a vital role in delivering patient-centered care and supporting our clients in their homes.

Key Responsibilities
  • Client Care: Provide nursing care to clients in their homes, including administering medications, wound care, and vital sign monitoring.
  • Assessment and Planning: Conduct thorough assessments of clients' physical and emotional needs, and develop personalized care plans to meet their unique requirements.
  • Collaboration and Communication: Work closely with clients, families, and healthcare professionals to ensure seamless care coordination and effective communication.
  • Documentation and Reporting: Maintain accurate and detailed records of client interactions, care provided, and any changes to their condition.
Requirements
  • Education: College diploma in Practical Nursing or equivalent.
  • Experience: Experience in home care or visiting care agency settings is an asset.
  • Certifications: Current CPR certification, licensure as a Registered Psychiatric Nurse, and First Aid certification.
  • Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic medical records.
