Job Title: Licensed Practical Nurse

VON Canada (Victorian Order of Nurses) is seeking a Licensed Practical Nurse to join our team in providing high-quality home and visiting care services to our clients.

Job Summary:

We are looking for a dedicated and compassionate Licensed Practical Nurse to provide care and support to our clients in their homes. The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide nursing care and support to clients in their homes, including administering medications, wound care, and vital sign monitoring.
  • Conduct home visits to assess clients' needs and develop care plans to meet their individual requirements.
  • Collaborate with other healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive care.
  • Document client information, including medical history, treatment plans, and progress notes.
  • Participate in ongoing education and training to stay up-to-date with best practices and industry developments.
Requirements:
  • Current licensure as a Licensed Practical Nurse (LPN) in the province of employment.
  • Post-secondary education in nursing, such as a diploma or degree in practical nursing.
  • Experience working in home and visiting care, preferably with a reputable organization like VON Canada.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on client-centered care.
