**Job Summary**

Discovery Point - Fayetteville is seeking a highly skilled Community Support Specialist to join its team in a dynamic and supportive environment. As a Community Support Specialist, you will play a vital role in providing exceptional support services to individuals with developmental disabilities and their families.

**Key Responsibilities:**

  • Act as the initial point of contact for individuals requesting services, representing their needs and completing intake assessments.
  • Collaborate with a multi-disciplinary team to provide support services, including case management and connecting individuals with community resources.
  • Assist individuals and families in accessing government services and other community supports.
  • Work closely with the Professional Resource Team to ensure effective service delivery and make decisions regarding service delivery.

**Requirements:**

  • A Social Service Worker Diploma or Social Worker Degree, or proof of registration with the Ontario College of Social Workers and Social Service Workers.
  • Two years' experience in the human services field, preferably with experience using various software programs.
  • A valid Class G driver's license and use of a vehicle.
  • Double COVID-19 vaccination.
