Southbridge Care Homes is currently in search of a Quality Improvement Nurse Manager to enhance the quality of care within our long-term care facilities. Reporting directly to the Executive Director and the Director of Care, the Quality Manager will play a pivotal role in overseeing and advancing quality improvement initiatives throughout the home. Your responsibilities will include evaluating the environment to pinpoint areas for enhancement that will elevate resident care and overall experience. You will serve as a driving force for quality and innovation by aligning various disciplines towards a common goal.

Key Responsibilities:
  • Lead the RNAO Best Practice Guidelines program.
  • Oversee quality initiatives with a focus on Balanced Score Card (BSC) indicators.
  • Implement project charters developed by regional teams.
  • Refine and evolve assigned project charters.
  • Engage with other Southbridge Homes to share implementation strategies.
  • Monitor and analyze quality indicator performance on a monthly basis.
  • Maintain and update quality improvement documentation regularly.
  • Participate in regional quality improvement discussions.
  • Collaborate with other quality improvement leaders to share successes.
  • Attend committee meetings related to BSC indicators.
  • Establish a core Quality Improvement committee with a focus on frontline staff.
  • Conduct environmental assessments to identify root causes of issues.
  • Engage with frontline team members to observe care practices.
  • Utilize various methodologies to analyze and understand problems.
  • Identify training gaps that affect quality and report findings to leadership.
  • Work with the Nursing Management team to hold regular professional practice meetings.
  • Introduce new care programs based on best practices.
  • Mentor and support junior management staff.
  • Communicate effectively with staff, residents, families, and external stakeholders.
  • Participate in nursing meetings to discuss new initiatives and quality improvements.
  • Chair the Quality Committee and collaborate with nursing leadership.
  • Oversee clinical programs within the Nursing Department, conducting audits and planning improvements.
  • Participate in strategic planning to align nursing practices with organizational goals.
  • Ensure policies align with the facility's culture and values.
  • Assist in developing and sustaining the Quality Improvement program.
  • Analyze clinical data to identify trends and lead quality improvement efforts.
  • Provide monthly updates on key performance indicators to senior management.
  • Evaluate resident programs for quality and effectiveness.
  • Engage with external stakeholders for collaborative efforts.
  • Maintain positive relations with regulatory bodies.
  • Participate in industry benchmarking activities.
Qualifications:
  • Minimum of 3 years of nursing supervisory experience, including long-term care.
  • Current certification as a Registered Nurse with the College of Nurses of Ontario.
  • Strong knowledge of accreditation processes and legislative requirements.
  • Proficient in Microsoft Office applications and quality improvement methodologies.
  • Demonstrated leadership and communication skills.
  • Strong organizational skills and attention to detail.
  • Ability to work flexible hours as needed.
  • Commitment to promoting person-centered care.
  • Knowledge of health and safety regulations.
