Key Responsibilities- Provide clinical leadership and supervision to designated staff, promoting excellence in client care and adherence to established standards and protocols.
- Facilitate effective communication and collaboration among team members, ensuring seamless coordination of care and services.
- Develop and implement care plans that address the unique needs and goals of clients, incorporating evidence-based practices and best available research.
- Collaborate with community agencies and healthcare providers to ensure comprehensive and coordinated care, promoting a seamless transition of care across settings.
- Conduct regular assessments and evaluations to identify areas for improvement, implementing strategies to enhance the quality and efficiency of care delivery.
The Health Promotion team at Vancouver Coastal Health is dedicated to providing exceptional care and support to clients in the community. As a Clinical Coordinator, you will be part of a dynamic and collaborative team that is passionate about delivering high-quality care and promoting healthy outcomes.
Requirements- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years' recent clinical experience in a relevant area of practice, with two (2) years' recent supervisory experience or an equivalent combination of training, education, and experience.
- Valid BC Driver's License required, as local area travel may require the use of a personal vehicle.
