Job Title: Pact Service Coordinator

Shared Health is seeking a skilled Pact Service Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for assessing, planning, and coordinating treatment, rehabilitation, and support services for program participants.

Key Responsibilities:
  • Assess and develop individualized treatment plans in collaboration with the clinical team.
  • Coordinate and provide services to individuals using the principles of psychosocial rehabilitation and recovery.
  • Work collaboratively with community agencies and healthcare providers to ensure comprehensive care.
  • Provide education and support to individuals and their families to promote mental health and well-being.
Requirements:
  • Minimum four years of related community experience working with people with serious and persistent psychiatric disabilities.
  • BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD, or related health/human service degree required.
  • Working knowledge of the principles of psychosocial rehabilitation and recovery.
  • Effective networking and agency relation skills.
  • Excellent organizational, communication, and interpersonal skills.
