About the Role

CarePartners is seeking a skilled Occupational Therapist to join our dynamic Rehabilitation Team in the Cambridge area. As a key member of our team, you will play a critical role in providing high-quality patient care and contributing to the growth of our therapy/rehabilitation program.

Key Responsibilities
  • Provide professional assessment of patients in the community and in their homes
  • Develop and maintain effective Care Plans with respect to Community Health Services
  • Work with patients to establish time-specific, measurable goals and outcomes in collaboration with the care team
  • Provide guidance and supervision to Community Support Workers in the field
  • Perform other duties as required
Requirements
  • Master's Degree in Occupational Therapy from a recognized university
  • Registration with the College of Occupational Therapists of Ontario
  • Proven computer skills, including email, MS Word, and Outlook
  • Strong assessment skills and ability to participate in effective treatment programs
  • Excellent English verbal and written communication skills
  • French language skills are an asset
  • Clear Background and Vulnerable Sector Check
