Job Summary
The Health Care Aide is an integral member of the home care team, working under the guidance of the Care Coordinator to deliver various personal care services tailored to meet the client's requirements.
Key Responsibilities
- Assisting with Personal Care / Activities of Daily Living (ADLs)
- Preparing meals
- Performing light housekeeping duties
- Accompanying clients on outings
- Running errands and shopping
- Providing medication reminders
- Assisting with client mobility and transportation
- Offering companionship to clients
- Adhering to care plans
- Maintaining documentation and charting
- Current clean background check or willingness to obtain one
- Ability to travel as required
- Access to internet and mobile phone
- Experience in palliative or hospice care is advantageous
- Experience in personal care and transfer assistance is beneficial
