Job Summary

LIFE READY FACILITATED CARE LTD is seeking a skilled Child and Youth Worker to join our team. As a Child and Youth Worker, you will play a vital role in supporting the rehabilitation and development of children and youth in our care.

Key Responsibilities
  • Case Management: Develop and implement individualized rehabilitation plans for children and youth, taking into account their unique needs and goals.
  • Program Development: Collaborate with colleagues to design and implement evidence-based rehabilitation programs that promote positive behavior change and skill development.
  • Supervision and Support: Provide guidance and support to children and youth, as well as their families, to help them navigate the rehabilitation process.
  • Community Engagement: Establish and maintain relationships with community partners and resources to ensure children and youth have access to necessary support services.
  • Documentation and Reporting: Maintain accurate and comprehensive records of client progress, including case notes, reports, and other relevant documentation.
Requirements
  • Education: Secondary (high) school graduation certificate
  • Experience: 7 months to less than 1 year
