Job Title: Occupational Therapist

CarePartners is seeking a highly skilled Occupational Therapist to join our dynamic Rehabilitation Team in Guelph and surrounding areas. As an Occupational Therapist, you will play a critical role in providing high-quality patient care and promoting independence in the home and community.

Key Responsibilities:
  • Provide professional assessment of patients in the community and in their homes
  • Develop and maintain effective Care Plans with respect to Community Health Services
  • Coordinate, supervise, and coach Community Support Workers in the field
  • Provide active guidance for workplace Health and Safety
Requirements:
  • Master's Degree (or equivalent) degree in Occupational Therapy from a recognized university
  • Proven computer skills, including email, MS Word, and Outlook, work scheduling systems
  • Excellent English verbal and written communication skills
  • French language skills are an asset
