Job SummaryWe are seeking a Clinical Care Partner to join our team at Sienna Senior Living. As a Clinical Care Partner, you will play a key role in providing follow-up and support to care community team members to ensure that care meets and/or exceeds organizational and provincial health authority requirements.
Key Responsibilities- Provide follow-up and support to care community team members to ensure that care meets and/or exceeds organizational and provincial health authority requirements.
- Assist in the development of compliance plans as required, and follow up on compliance action plans in consultation with the care community team members.
- Lead care community and organization-wide improvement processes/projects as directed and in consultation with the Regional VP Operations and care community team members.
- Assist and coach care community team members at all levels to improve processes, strategies, and activities concerning care and services through the provision of education and orientation.
- Provide recommendations regarding training and education that would benefit the care communities and the organization as a whole.
- Post Baccalaureate education in Nursing, Health Sciences, or a related field is preferred.
- Preference will be given to candidates with Director of Care experience.
- Knowledge and understanding of provincial regulatory requirements and the inspection process.
- 3 years of related management and relevant experience in long-term care.
- Able to drive and/or travel to long-term care communities - access to a reliable vehicle and valid G licence is required.
