Clinical Care Partner

We are seeking a highly motivated and experienced Clinical Care Partner to join our Support Services Office team. As a Clinical Care Partner, you will play a vital role in ensuring that our care communities meet and exceed the organization's and provincial health authority requirements.
Key Responsibilities- Collaborate with the Regional VP, Executive Director, and DOC to provide follow-up and support to all care community team members.
- Assist in the development of compliance plans as required, and follow up on compliance actions plans in consultation with the care community team members.
- Support the development of processes and evidenced-based practices to improve care and services.
- Lead care community and organization-wide improvement processes/projects as directed and in consultation with the Regional VP Operations and care community team members.
- Coach care community team members at all levels to improve processes, strategies, and activities in relation to care and services.
- Provide recommendations regarding training and education to benefit the care communities and the organization as a whole.
- Work in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
- Must be a Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
- Post-Baccalaureate education in Nursing, Health Sciences, or a related field is preferred.
- Preference will be given to those candidates with Director of Care experience.
- Certificate of Gerontological Nursing (Canadian Nurses Assoc.) preferred.
- Knowledge and understanding of provincial regulatory requirements and inspection process.
- 3 years related management and relevant experience in long-term care.
- Evidence of ongoing professional development and a demonstrated commitment to continuous learning.
- Highly motivated; demonstrates an ability to be self-directed, working independently and as a member of a collaborative team.
