About the Role
We are seeking a skilled Mental Health Support Worker to join our team at the Provincial Assessment Centre. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care and support to our clients.
Key Responsibilities
- Monitor clients' progress and provide emotional support and counseling as needed.
- Participate in the admission, case planning, and discharge process for clients, including conducting interviews and gathering relevant information.
- Establish rapport with clients and provide support with daily living activities, such as time management and interpersonal communication.
- Coordinate services with medical professionals and community resources, and act as an advocate for clients.
- Plan and lead groups, such as sports and exercise, social support groups, and social activities, to support client skill development.
- Prepare and maintain client charts, including updating files and completing standard forms.
- Maintain a safe and clean environment for clients and team members.
Requirements
- Diploma from a recognized Social Services Support Worker educational program, or equivalent combination of education and experience.
- Two years' recent, related experience, or an equivalent combination of education and experience.
- Valid driver's license and ability to travel locally.
