We are seeking two highly skilled and compassionate individuals to support the Medical Assistance in Dying program at Trillium Health Partners.
Key Responsibilities- Intake and Processing of Requests for Medical Assistance in Dying (MAID) - responding to inquiries and referrals from patients and healthcare providers in a sensitive and respectful manner.
- Education, Coaching, and Mentorship for MAID - providing education, coaching, and mentorship to referring physicians/NPs, patients and family members, healthcare teams, and provision team members about all aspects of MAID process.
- Implementation, Operation and Evaluation of MAID - coordinating and overseeing provision of MAID in accordance with hospital policies and procedures and applicable legal and professional standards.
- Regulated Health Professional in good standing with the relevant professional College.
- Experience supporting patients and family members at end of life.
- Effective and empathic verbal and written communication skills.
- Excellent organization and time management skills.
- Commitment to collaborative inter-professional practice.
