Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team in the XXX area. As a member of our community nursing team, you will have the opportunity to work in a variety of settings, including in-home, clinic, retirement home, or school, providing exceptional nursing services to patients and their families.
Key Responsibilities- Develop and maintain effective care plans with respect to community health services, independently or in cooperation with the Home and Community Care Support Services (HCCSS).
- Provide nursing services in a variety of settings, including in-home, clinic, retirement home, or school.
- Communicate and liaise with patients, family members, and others within the interdisciplinary care team.
- Work with patients to provide exceptional nursing services, both in-person and virtually.
- Travel regularly to provide services in various settings.
- A valid driver's license and vehicle.
- Excellent English verbal and written communication skills.
- A current immunization record, including a 2-step TB test.
