About the Role

Pacific Coast Community Resources is seeking a dedicated and compassionate Community Support Worker to join our team. As a Community Support Worker, you will play a vital role in providing care and support to individuals living in staffed homes within their community.

Your primary responsibility will be to empower and support individuals to live independently and self-determiningly, while maintaining excellent standards of care, health, and safety. You will work collaboratively with a multidisciplinary team to ensure the rights and choices of the individuals we support are respected and advocated for.

Key Responsibilities
  1. Ensure the rights and choices of individuals are respected and advocate as needed.
  2. Interact effectively with individuals we support.
  3. Empower individuals to be full citizens of their community.
  4. Participate in the development, implementation, and documentation of Personal Service Plans.
  5. Foster positive relationships with family members, professionals, co-workers, and community members.
  6. Assist with activities of daily living and medication administration.
  7. Record daily activity and personal health information (PHI) of individuals we support on our case management database.
  8. Work with individuals to ensure their home is clean and well maintained, including daily food preparation and light housekeeping duties.
Requirements
  1. Grade 12 education.
  2. Community Support Worker Diploma or equivalent.
  3. Minimum 2 years of experience in community social services/health care/caregiver.
  4. Experience in Dysphagia.
  5. Experience in using mobility equipment and safety devices.
  6. Experience in Bipap Management, Oxygen Administration, Seizure Management, and Airway Management.
  7. Current Emergency First Aid/CPR and Food Safe certificate.
  8. Previous behaviour management course/training.
  9. Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members.
  10. Non-verbal communication skills.
  11. Valid Class 5 Driver's Licence.
  12. Satisfactory Driver's Abstract.
  13. Criminal record clearance for children and vulnerable adults.
  14. Proof of COVID-19 vaccination status as per current Public Health Orders.
  15. TB Test.
  16. Moderate computer literacy.
  17. Eligibility to work in Canada.
