Job Summary

We are seeking a highly skilled and compassionate Social Work Specialist to join our team at Halton Healthcare. As a key member of our multidisciplinary team, you will provide clinical social work and discharge planning services to inpatients on our Complex Transitional Care Units.

Key Responsibilities
  • Conduct Comprehensive Assessments: Conduct thorough social work and psychosocial assessments on inpatients, utilizing a variety of modalities and adhering to professional standards, clinical policies, and practices.
  • Develop and Implement Care Plans: Coordinate, formulate, document, and implement care plans for hospital discharge and safe transition back to the community or institutional setting.
  • Participate in Interdisciplinary Rounds: Participate in team conferences, educational rounds, and other interdisciplinary activities to ensure comprehensive care.
  • Provide Support and Education: Provide support to patients and families to aid in adjustment related to health issues and resolve complex interpersonal concerns. Educate patients and families regarding relevant healthcare legislation and policies.
  • Provide Therapeutic Interventions: Provide selected therapeutic interventions, supportive counseling, community outreach, and advocacy to patients and families.
  • Coordinate Community Resources: Provide guidance regarding community resources, finances, legal issues, workplace injuries, and other relevant topics.
Qualifications
  • Education and Certification: Bachelor of Social Work (BSW) or Master of Social Work (MSW) degree required. Current registration and membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is essential.
  • Experience and Skills: Experience providing scope of practice social work to patients within an acute care or post-acute hospital setting. Strong knowledge of community resources and ability to work collaboratively with community partners. Familiarity with relevant healthcare legislation and policies.
  • Personal Qualities: Demonstrated ability to maintain an evidence-based practice approach. Strong critical thinking and problem-solving skills. Excellent communication and interpersonal skills.
