Key Responsibilities:

  • Perform initial evaluations of care requirements and develop personalized care plans for new patients
  • Ensure that the living conditions of new patients comply with safety regulations
  • Deliver initial personal care services to new patients
  • Advise on the scheduling of ongoing patient visits based on service needs
  • Communicate company policies and scheduling procedures to patients and their families
  • Maintain precise and up-to-date patient records
  • Respect the cultural needs, privacy, and confidentiality of both patients and staff
  • Stay informed about changes in practice settings that may impact the scope of care

Qualifications:

  • Registered and in good standing as an RPN/LPN with provincial regulatory bodies
  • A minimum of two years' experience in nursing practice

  • Excellent communication and organizational abilities
  • Proficient in patient-centered care principles and practices
  • Familiarity with available community resources
  • Strong independent critical thinking and problem-solving skills
  • Clear criminal background check with Vulnerable Position Screening required
  • Access to a personal vehicle
