Key Responsibilities:
- Perform initial evaluations of care requirements and develop personalized care plans for new patients
- Ensure that the living conditions of new patients comply with safety regulations
- Deliver initial personal care services to new patients
- Advise on the scheduling of ongoing patient visits based on service needs
- Communicate company policies and scheduling procedures to patients and their families
- Maintain precise and up-to-date patient records
- Respect the cultural needs, privacy, and confidentiality of both patients and staff
- Stay informed about changes in practice settings that may impact the scope of care
Qualifications:
- Registered and in good standing as an RPN/LPN with provincial regulatory bodies
- A minimum of two years' experience in nursing practice
- Knowledge of relevant legislation and best practice standards in community healthcare
- Excellent communication and organizational abilities
- Proficient in patient-centered care principles and practices
- Familiarity with available community resources
- Strong independent critical thinking and problem-solving skills
- Clear criminal background check with Vulnerable Position Screening required
- Access to a personal vehicle
