About the Role

We are seeking a highly skilled Care Coordinator to join our team at ParaMed Home Health Care. As a Care Coordinator, you will be responsible for matching clients in need of care with appropriate field staff, providing exceptional customer service skills and ensuring seamless care delivery.

Key Responsibilities

  • Assign staff to patients based on various factors, including patient preferences, staff availability, and skill sets.
  • Build and manage patient care schedules.
  • Enter and update employee and patient data within our computerized system.
  • Respond professionally to calls and electronic communication from funders, staff, patients, and families.
  • Document and report all staff and patient interactions.

Requirements

  • One year of administrative or office experience is an asset.
  • Scheduling experience is an asset.
  • Strong computer skills.
  • Superior customer service skills.
  • Ability to communicate effectively both orally and in writing.
  • Compassion and empathy when interacting with patients and their families.

Working Hours

Full-time position available.
