  • Coordination: Coordinate services and resources to ensure seamless transitions and optimal outcomes for clients.
  • Communication: Maintain effective communication with clients, families, and healthcare professionals to ensure that needs are met and plans are implemented.
  • Problem-Solving: Identify and address potential problems or barriers to care, and develop solutions to ensure optimal outcomes.

Requirements:

  • Education: Bachelor's degree in a relevant field, such as nursing, social work, or occupational therapy.
  • Experience: Minimum 2 years of experience in a related field, with a strong understanding of healthcare systems and community resources.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Knowledge: Strong knowledge of healthcare systems, community resources, and relevant legislation and policies.

Working Conditions:

This role requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The successful candidate will be able to work effectively in a team environment, with strong communication and interpersonal skills.

Language: Fluency in English and French is required.
