POSITION SUMMARY:
The Geriatric Emergency Management (GEM) Nurse serves as a vital clinical resource dedicated to the assessment and treatment of frail elderly individuals. This role emphasizes the care of seniors who are vulnerable to adverse health events, loss of autonomy, and potential hospital or long-term care admissions.
KEY RESPONSIBILITIES:
- Provide specialized assessments and treatment services tailored to the needs of frail seniors.
- Strengthen connections between hospital services and community resources while advocating for the elderly population.
- Act as an educator and capacity builder in geriatrics within the Emergency Department and broader community, ensuring timely and appropriate care for at-risk seniors.
- Facilitate the prompt discharge of elderly patients from the Emergency Department, linking them to necessary community support services.
- Promote best practices in elder care nursing and foster an elderly-friendly approach within the Emergency Department.
- Utilize the Standards of Nursing Practice and Nursing Process effectively in the assessment, planning, implementation, and evaluation of patient care.
- Assist in planning corrective measures to enhance safety for both patients and staff.
- Adhere to all relevant legislation, organizational policies, and departmental regulations concerning occupational health and safety.
REQUIRED QUALIFICATIONS:
- Certificate of Competence from the College of Nurses of Ontario.
- Geriatric Resource Nurse Certification or equivalent geriatric education.
EXPERIENCE:
- A minimum of three years of experience working with frail geriatric patients.
- At least three years of experience conducting assessments of geriatric patients.
SKILLS:
- Advanced competence and knowledge in community services focused on frail elderly individuals.
- Strong interpersonal, oral, and written communication skills, along with effective presentation abilities.
ABILITIES:
- Proficient in the use and care of all mechanical equipment necessary for patient care.
- Ability to collaborate effectively within an interdisciplinary team.
- Demonstrated competency in physical assessments and assisting with patient mobilization.
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint).
- Proven ability to maintain positive working relationships and interact with individuals in a professional and courteous manner, both in person and via telephone.
- Exhibit behavior consistent with the organization's mission, vision, and core values.
