Northern Health Authority is seeking a skilled Registered Nurse to join our team in Primary Care. As a Primary Care Nurse, you will be responsible for providing high-quality patient care, developing and implementing care plans, and collaborating with interdisciplinary teams to ensure seamless care delivery.
Key Responsibilities:
• Provide direct patient care, including assessments, diagnoses, and treatment plans
• Develop and implement care plans in collaboration with patients, families, and healthcare teams
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• Participate in quality improvement initiatives to enhance patient outcomes and experiences
Requirements:
• Current registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant
• Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
• Current valid BC Driver's License
Preferred Qualifications:
• Experience working in a primary care setting
• Knowledge of adult learning principles and facilitation skills
• Ability to work effectively in a team environment
