Job Summary
The Delaware Department of Health and Social Services (DHSS) is seeking a skilled Nurse Care Specialist to join our team. As a key member of our healthcare team, you will provide high-quality nursing care to our residents, including direct nursing care, assessments, and coordination of healthcare services.
Key Responsibilities
- Provide or evaluate direct nursing care to residents, including administering medications, wound care, and vital sign monitoring.
- Develop, implement, and review nursing care plans to ensure individualized care for each resident.
- Provide health education and guidance to residents, families, and healthcare staff on managing healthcare needs.
- Maintain effective working relationships with multidisciplinary teams, including physicians, nurses, and other healthcare professionals.
- Prepare and submit accurate records, reports, and evaluations to ensure compliance with regulatory requirements.
- Participate in quality assurance activities to improve patient outcomes and satisfaction.
Requirements
- Possession of a Delaware Registered Nurse license, temporary license, or multi-state compact license.
- Education, training, and/or experience demonstrating competence in areas such as patient assessment, medication management, and wound care.
Preferred Qualifications
- Bachelor's degree or higher in Nursing.
- At least one year of experience as a Registered Nurse.
