Job Summary

We are seeking a highly motivated and compassionate Life Skills Support Worker to join our Primary Care team at Northern Health Authority. As a key member of our interprofessional team, you will play a vital role in providing exceptional care and support to individuals with complex health needs.

Key Responsibilities
  • Apply best practices to provide person-centered care and support to individuals with complex health needs.
  • Work collaboratively with the interprofessional team to develop and implement individualized care plans.
  • Provide education and training to individuals and their families to promote independent living and self-management.
  • Conduct regular assessments and evaluations to identify changing needs and develop strategies to address them.
  • Collaborate with community resources and services to ensure seamless care transitions and coordination.

Requirements
  • Diploma from a recognized Social Services Support Worker educational program, or Residential Care Aide program, with two years of recent experience working with individuals with complex health needs.
  • Or, Grade 12 plus a Community Mental Health Worker Certificate and Addictions Studies Certificate, with three years of recent experience working with adults with mental illness and/or addictions diagnosis.
  • Valid driver's license and First Aid Certificate, CPR, and Food Safe certification.
Skills and Abilities
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and healthcare teams.
  • Strong problem-solving and conflict resolution skills, with the ability to analyze complex situations and develop effective solutions.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
  • Basic computer skills, including Microsoft Office and electronic medical records.
