Key Responsibilities- Apply best practices to provide person-centered care and support to individuals with complex health needs.
- Work collaboratively with the interprofessional team to develop and implement individualized care plans.
- Provide education and training to individuals and their families to promote independent living and self-management.
- Conduct regular assessments and evaluations to identify changing needs and develop strategies to address them.
- Collaborate with community resources and services to ensure seamless care transitions and coordination.
- A comprehensive benefits package, including extended health and dental coverage, and a municipal pension plan.
- Three weeks of vacation time with one year of continuous service.

- Diploma from a recognized Social Services Support Worker educational program, or Residential Care Aide program, with two years of recent experience working with individuals with complex health needs.
- Or, Grade 12 plus a Community Mental Health Worker Certificate and Addictions Studies Certificate, with three years of recent experience working with adults with mental illness and/or addictions diagnosis.
- Valid driver's license and First Aid Certificate, CPR, and Food Safe certification.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and healthcare teams.
- Strong problem-solving and conflict resolution skills, with the ability to analyze complex situations and develop effective solutions.
- Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
- Basic computer skills, including Microsoft Office and electronic medical records.
