Job Summary

Cornwall Community Hospital is seeking a skilled Social Work Specialist to provide comprehensive care to individuals affected by sexual assault and intimate partner violence. As a key member of our team, you will embody a culture of caregiver inclusion and develop a patient-centered plan of care.

Key Responsibilities

• Provide trauma-informed care to patients, prioritizing their physical and emotional well-being
• Develop and implement individualized care plans, incorporating patient goals and preferences
• Collaborate with multidisciplinary teams to ensure seamless care coordination
• Provide education and support to patients and their families, promoting empowerment and self-advocacy
• Participate in quality improvement initiatives, contributing to the enhancement of patient care and services
• Maintain accurate and confidential records, adhering to hospital policies and procedures
• Stay up-to-date with best practices in trauma care, incorporating evidence-based interventions into practice
• Foster a culture of inclusivity and respect, promoting diversity and equity in patient care

Requirements

• Master's Degree in Social Work
• Current registration with the Ontario College of Social Workers and Social Service Workers
• Minimum of 3 years' experience in trauma care, with demonstrated expertise in crisis intervention and case management
• Valid driver's license and access to own transportation
• Proven ability to manage crisis situations, prioritizing patient safety and well-being
• Strong written and communication skills, with ability to develop a community network
• Ability to work independently, with strong problem-solving and critical thinking skills
• Good computer skills, with proficiency in electronic medical records and other relevant software
• Demonstrated regular, punctual attendance and commitment to patient safety
