Job Title: Home Care Scheduling Clerk III

Job Summary:

The Home Care Scheduling Clerk III is a key member of our interdisciplinary team, responsible for scheduling and assigning client visits by the Home Care Direct Service Nurse. This role requires strong organizational skills, attention to detail, and effective communication with clients, staff, and other stakeholders.

Key Responsibilities:
  • Maintain accurate and up-to-date employee schedules in our Electronic Scheduling Program.
  • Respond to service requests and changes in services from the Home Care Case Coordinator and/or Direct Service Nurse, identifying available staffing resources.
  • Process service requests according to priority, consulting with the Home Care Case Coordinator and/or DSN as needed.
  • Provide assigned workers with assignment information and notify them of service resumption or termination.
  • Communicate with clients regarding scheduling as needed and maintain accurate Home Care client information in accordance with established guidelines.
  • Complete timekeeping responsibilities as applicable and adhere to PHIA and FIPPA legislation.
  • Document concerns and refer them to the respective supervisor.
  • Respond to calls from staff and plan and coordinate replacement staff as required, consulting with the Supervisor as needed.
  • Refer employee performance concerns to the respective supervisor and identify self-learning needs as required.
  • Work within the scope of the position and perform other duties as assigned.
